What is a family integrator?

A family integrator is the family member who became the unofficial coordinator, holding appointments, medications, authorizations, and the next handoff. This person keeps a single picture of care when more than one person or provider is involved.

What the role involves

Appointments, documents, medications, and the next handoff all need a place in the plan. The family integrator holds those details together so the next caller can understand what is happening and who owns the next step.

That includes keeping the medication list current, gathering authorizations and important documents, and making the family’s responsibilities visible. The work continues between appointments and after care changes, when the first emergency has passed.

Where it breaks down

Care can break down between units, between organizations, and between the official plan and the person who has to carry it. Each organization may complete its part while the family is still left holding continuity that no institution scheduled.
