The treating team provides discharge instructions, treatment guidance, follow-up, and the clinical contact for questions. Ask for written directions before leaving.
The family
Family members can organize the home, transportation, meals, appointments, and updates. Give each recurring task an owner so one person does not end up carrying the whole plan.
The home care provider
OPA can discuss practical support, scheduling, and the household routine. The team can work alongside the discharge plan without taking over clinical decisions.
