Before discharge, ask the hospital team who to call with questions, what follow-up is planned, and what changes to daily routines are expected. Bring home written instructions and share them with the people who will be helping.
Prepare the household
Think through meals, transportation, mobility around the home, errands, and who will be present at different times of day. Decide how family members will share updates without placing the whole load on one person.
