WHAT IS AGING IN PLACE?

Aging in Place allows seniors to comfortably and healthily remain in their homes throughout the continuum of care. It ensures that after hospitalizations or time in skilled nursing facilities they can be discharged to their home with the help of caregivers from home care agencies, that assist them with the services needed based on their care plan.
